Findings
The Alleged Victim (AV) relies on the facility for his/her care, is a known fall risk, and is care planned for assistance with dressing. According to an investigation, on more than on occasion AV was found with inappropriate clothing or footwear, placing AV at risk for falls. The facility failed to ensure the care plan was followed, which is a violation of resident rights, is neglect of care, and constitutes abuse.
