Findings
The facility failed to implement appropriate interventions and failed to appropriately care plan related to the Alleged Victim’s (AV) known history of falls. On or about November 29, 2023, AV suffered a fall and was diagnosed with a fractured left hip, causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
