Findings
The facility failed to implement interventions, appropriately care plan and monitor Witness 1 (W1) according to his/her aggression and prior resident altercations. The failure resulted a physical altercation with the Alleged Victim, causing unreasonable discomfort and risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
