- The Alleged Victim (AV) has his/her medication administered by the facility. On or about December 16, 2023, AV complained of constipation and needed to receive a suppository. Many of AV's medications are crushed and placed into yogurt for AV, however, AV's suppository medication is to be given rectally. On this day, Alleged Perpetrator #2 (AP2) gave AV his/her suppository medication crushed in yogurt. AV questioned AP2 about it, however, AP2 assured AV it was correct and to take the medication. As a result of taking the medication orally, AV suffered stomach upset, stomach pain and significant anxiety. The facility's failure to ensure AP2 knew how to administer medications correctly placed AV in pain and at risk for serious harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
