Findings
On or about December 16, 2023, the facility failed to follow Alleged Victim's (AV) care plan by not providing scheduled toileting and necessary care, resulting in AV being left in soaked briefs and experiencing unreasonable discomfort while already having a skin breakdown related to pressure injury. The facility's failure is a violation of resident rights, is considered neglect of care which constitutes abuse.
