Findings
Alleged Victim (AV) relies on the facility for his/her care. Alleged Perpetrator 2 (AP2) was reported to have physically abused AV on two (2) occasions. It was stated that AP2 yelled at AV, grabbed AV by his/her collar, and forcefully directed AV to his/her room. AP2 did not follow the service plan with regard to cares. AP2 failed to protect AV from emotional abuse, which is neglect of care and constitutes abuse.
