Findings
On or about December 15, 2023, Alleged Perpetrator #2 (AP2) was assisting the Alleged Victim (AV) with cares when AV became agitated and began hitting and scratching. AP2 requested that Alleged Perpetrator #3 (AP3) assist him/her with cares of AV. AP2 asked AP3 to hold AV's hands so AP2 could finish with AV's care needs. Later that evening, AV was found with a large bruise on his/her wrist. AP2 did not attempt any interventions or re approaching AV to de-escalate the situation. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. AP3's actions were found to be not substantiated in this case. The facility's failure is a violation of Oregon Administrative Rules.
