- Witness #6 (W6) is known to engage in aggressive behaviors and altercations with residents and staff. W6 is currently on one to one monitoring during waking hours, which is typically only day shift. There is not enough staff available on night shift to complete one to one care for W6. On or about December 18, 2023, W6 was wandering around the common area at approximately midnight. W6 entered the Alleged Victim's (AV) room and yelled at AV to get out of his/her bed and hit AV in the chest. Staff intervened and removed AV from the room, however, AV was scared and in pain from being hit in the chest by W6. The facility failed to provide a safe environment for AV, causing unreasonable discomfort. The facility's failure to ensure resident safety is a violation of resident rights, is considered neglect of care and constitutes abuse.
