Findings
Alleged Victim (AV) relies on the facility for his/her care. Alleged Perpetrator 2 (AP2) was reported to be using excess force while performing transfers and cares which resulted in AV exclaiming, he/she was being hurt. AP2 was witnessed continuing to transfer AV and not re-approach after AV indicated he/she did not want to transfer at that time. AV was heard saying "No Stop, you are causing me pain!", during cares with AP2. AP2 failed to protect AV from physical abuse, which is neglect of care and constitutes abuse.
