- The Alleged Victim (AV) relies on the facility for care, and is care planned for assistance with transfers and movement throughout the facility. According to an investigation, on or about December 23, 2023, the Alleged Perpetrator 2 (AP2) put AV in bed, despite AV stating they were not tired and did not want to lay down. AV was unable to get out of bed by themselves and stayed in bed crying. AP2 involuntarily secluded AV, which is a violation of resident rights, is considered neglect of care and constitute abuse. The facility failed to ensure resident rights/treatment was being followed and resident was protected from involuntary seclusion which is a violation of Oregon Administrative Rules.
