Findings
The Alleged Victim (AV) relies on the facility for care, and is a high fall risk due to cognitive decline and an unsteady gait. AV is service planned to have frequent location checks, and require redirection often as he/she is known to stand up and attempt to ambulate without his/her assistive device. On or about December 27, 2023, AV had a fall resulting in a head laceration that required steri-strip placement. The facility failed to provide services, including implement effective interventions with appropriate staffing, resulting in repeated unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
