Findings
The facility failed to follow the care plan and provide appropriate services according to the Alleged Victim (AV)'s needs, relating to modified diet. AV is care planned to be positioned at a ninety-degree angle while eating, drinking, or taking medication. According to an investigation, meals were left on AV’s tray while he/she was lying in bed, which posed a serious risk of harm. This failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
