Findings
On or about December 20, 2023, Alleged Perpetrator 2 (AP2) failed to provide a safe medication administration system. AP2 administered the wrong amount of medication to The Alleged Victim (AV), resulted in AV experienced unreasonable discomfort. AP2's actions is a violation of resident rights, is considered neglect of care and constitute abuse, which is a violation of Oregon Administrative Rules.
