Findings
According to 411-054-0300(11), the facility must have a functioning call system intended to alert staff of resident's needs. On or about January 3, 2024, AV used his/her call light and it took staff approximately 90 minutes to respond to his/her call light, placing AV at risk of harm. The failure is a violation of Oregon Administrative Rules.
