Findings
The alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim's care plan to provide assistance with transferring/ambulation. The failure resulted in AV experiencing a fall and was transported to the hospital for treatment. AP2's actions are considered neglect of care and constitutes abuse. The facility's failure to ensure AV's care plan was followed is a violation of Oregon Administrative Rules.
