Findings
The Alleged Victim (AV) relies on the facility for his/her care. According to an investigation, on multiple occasions, AV was not given medication as ordered and medical orders were not followed, placing AV at risk of harm. The facility failed to provide a safe medication system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
