Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about December 2, 2023, AV was found on his/her knees at the end of their bed with his/her wheelchair about 10 feet away. AV stated he/she was trying to get out of bed, which lead to his/her fall. The facility failed to provide a safe environment for AV putting AV in risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
