Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about November 5, 2023, AV was attempting to go to the bathroom when they fell in the doorway of their room. As a result, AV sustained a laceration to the scalp. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
