Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about December 23, 2023, AV sustained a fall in the doorway leading to AV sustaining an injury to his/her head. The facility failed to provide a safe environment for AV putting AV in risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
