Findings
On or about September 22, 2023, it was noticed by staff that the Alleged Victim (AV) had a sore on his/her backside. Staff noted the information in the progress notes, however, no one notified the facility Nurse of the wound. The facility Nurse found out about the wound on October 9, 2023 and contacted AV's physician and ordered wound care from home health, as well as wound care from facility staff. From September 22, 2023 to October 9, 2023, AV's wound worsened, causing unreasonable discomfort and risk of serious harm. The facility's failure to provide timely wound care and respond to a change of condition is a violation of resident rights, is considered neglect of care and constitutes abuse.
