Findings
According to the documentation, the Alleged Perpetrator (AP2) failed to follow the Alleged Victim’s (AV) service plan when the AV required assistance with transferring. The failure resulted in the AV suffering a fall and received a skin tear, which is a violation of resident rights and is considered abuse. The facility failed to follow the care plan which is a violation of Oregon Administrative rules.
