- The facility failed to follow Alleged Victim's (AV) physician orders for a chest x-ray and oxygen that facility received on January 15, 2024. Based on facility documentation and interviews, AV did not have a chest x-ray until January 29, 2024, and AV did not receive oxygen until January 19, 2024. The facility's failure to ensure a service was provided is a violation of Oregon Administrative Rules.
