- Alleged Victim (AV) has a history of becoming combative to care. On or about February 01, 2024 Alleged Perpetrator #2 (AP2) and another care staff assisted in providing care to av. Av became combative to care. Care staff reported witnessing AP2 hit AV's arm in reaction to AV's combativeness. AP2 submitted a letter of resignation on February 2, 2024 to the facility. AP2 admitted to the facility in writing that AP2 reactively hit AV out of defense due to AV's combative nature while providing care to AV. AP2 had been properly trained on dementia care and how to navigate combative patient care. AP2’s actions are a violation of resident rights, are considered neglect of care and constitute physical abuse. The facility failed to ensure AV was protected from physical abuse which is a violation or Oregon Administrative Rules.
