Findings
On or about January 22, 2024, the Alleged Victim (AV) suffered a vascular event which affected his/her ability to ambulate. On or about January 24, 2024, AV was found on the floor on the ground next to his/her bed. AV suffered a broken leg during this fall. AV's care plan was not updated after his/her vascular event to include clear direction to staff regarding Av's transfer and ambulation requirements after his/her significant change of condition. The facility's failure to have oversight and monitoring of AV's change of condition is a violation of resident rights, is considered neglect of care and constitutes abuse.
