- On or about January 29, 2024, Witness 1 (W1) was found in Alleged Victim's (AV’s) room with h/h pants down. The AV was observed as being fearful, and complained of pain on h/h bottom. The AV was checked out but no injuries or marks were found. Sexual abuse on or about January 29, 2024 from W1 to AV could not be ruled out based on the information gathered. During the days that followed the AV also displayed a significant change in behavior towards the W1, showing obvious fear and urinated on h/h-self in at least 2 occasions. The report of the incident in question was deleted by Alleged Perpetrator #2 (AP2). Deleting the incident report of possible sexual abuse placed the AV in further harm. Despite the report being deleted, management was made aware of incident in question that occurred on or about January 29, 2024 by staff. Management was notified of the AV’s change in behavior towards the W1 as showing obvious fear by shaking and urinating on themselves. Despite management being aware of possible sexual abuse that occurred on or about January 29, 2024 and the AV’s change in behavior, management failed to place interventions to help keep the AV safe and thus further placing the AV at risk of serious harm. AP2's and the facility actions failed to ensure a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse
