Findings
On or about January 22, 2024, the Alleged Victim (AV) and Witness #1 (W1) engaged in an altercation when AV said hello to W1, W1 slapped AV on the arm. AV did not suffer any injury, however, AV was placed at risk of harm. W1 was agitated and W1 is care planned to have interventions when W1 is having behaviors. Staff did not intervene when W1 was having behaviors. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
