Findings
The Alleged Victim (AV) has a history of falls, is known to self-transfer without assistance, and is care planned for a tab alarm in wheelchair. According to an investigation, AV was in his/her wheelchair without the alarm and fell while attempting to self-transfer, which resulted in a skin tear. The facility failed to follow AV’s care plan, which is a violation of resident rights, is neglect of care which constitutes abuse.
