- The Alleged Victim (AV) and Witness #1 (W1) are known to have altercations with each other and are care planned to be kept separated and observe if one or both become agitated and intervene and redirect from the situation. On or about December 15, 2023, AV was sitting in W1's chair when staff heard yelling and saw W1 walking away from AV. AV had scratches on his/her cheek, neck and arm. W1 may have suffered bruising to his/her arm. The facility's failure to follow the care plan and prevent an altercation is a violation of resident rights, is considered neglect of care and constitutes abuse.
