Findings
According to the documentation, the facility failed to properly care plan for the Alleged Victim’s (AV) ability to toilet themselves. On or about February 11, 2024, the AV was seen by a medical professional, and it was determined the AV was wearing clothes saturated with urine for an undetermined amount of time. The medical professional determined the AV was incontinent with toileting and required assistance with toileting an hygiene needs. AV’s service plan indicates the AV does not require assistance with toileting. The failure to properly care plan left the AV in urine-soaked clothes, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
