Findings
The Alleged Victim (AV) had a fall in December 2023 with injury. AV's care plan wasn't updated to reflect AV as a fall risk. AV fell again on January 25, 2024 while he/she was trying to get out of bed. The facility failed to properly care plan to avoid falls. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
