Findings
AV is to be kept in line of sight of staff while in common areas and out of arms reach of other residents however, the facility leaving all pod doors open for AV to wander makes this care planning ineffective as while wandering in the common areas there are no assigned staff to monitor the common areas. The failure places AV at risk of harm due to history of altercations, which is a violation of Oregon Administrative Rules.
