Findings
On or about February 25, 2024, the Alleged Victim (AV) was care planned for safety checks three times per shift, due to new medication which could cause dizziness. On or about February 26, 2024, AV suffered a fall and was sent to the hospital where he/she was diagnosed with an orbital and shoulder fracture. There is no documentation showing that the facility staff had completed any safety checks on AV on February 26, 2024. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
