- The facility failed to provide appropriate supervision to the Alleged Victim (AV) according to his/her needs. On or about March 6, 2024, AV eloped from the secured unit of the facility and was found in the lobby of the unsecured part of the facility with injuries to his/her lip, elbow, hands, and knees. Based on facility documentation and interviews, after this incident, the care staff checked all doors in the secured unit of the facility and found that one of the emergency exit doors to the back of the building was unlocked. Charge staff observed the push bar on the emergency exit door sticking, preventing the door from latching/locking, and submitted a maintenance request to have the door fixed. The facility's failure resulted in AV eloping the secured building, without staff knowledge, placing him/her at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
