Findings
The Alleged Victim (AV) relies on the facility for his/her care and has a history of removing feces from their incontinent brief and eating it. On or about March 7, 2024, AV was found on the floor of their room with feces on their hands, arms, legs, and face. The facility failed to properly care plan resulting in a loss of personal dignity, which is violation of resident rights, is considered neglect of care and constitutes abuse.
