Findings
The facility failed to follow the Alleged Victim’s (AV) care plan to provide one on one interventions when AV is in his/her wheelchair. On several occasions, AV was left unattended in his/her wheelchair and experienced multiple falls causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
