Findings
The Alleged Victim (AV) was a known fall risk and care planned for monitoring for transferring and ambulation and staff to have a visual on AV in the gathering areas. According to an investigation, on or about March 19, 2024, AV experienced an unwitnessed fall in the dining area, resulting in an injury to his/her eyebrow and bridge of the nose. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care which constitutes abuse.
