- Witness 1 (W1) has a history of being agitated with other residents and staff, and being involved in resident-to-resident altercations . W1 will yell at staff and other residents. W1 and Alleged Victim (AV) have only been in verbal altercations in the past. W1's service plan requires interventions if W1 becomes agitated or angry, staff are to redirect W1 by attempts to do an activity or task, offer a snack or drink, attempt to provide care 3 times, and then wait 10 minutes and try a different caregiver. On or about March 20, 2024, AV was leaving the dining area after breakfast and AV entered the shared room of AV and W1, W1 became agitated with AV, yelling at AV, and W1 grabbed AV's walker and pushed AV to the floor in a shared room. As a result of the resident-to-resident altercation, AV had a bruise and skin tear on AV's left upper wrist. The facility failed to provide a safe environment for AV and follow W1' s care plan interventions, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
