Findings
The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs. AV has a history of rashes. AV is service planned for full assistance with incontinence care, and showering. AV's service plan states that AV is independent in skin care; but the service plan only provides a generic intervention of reporting all new skin issues to a med tech with no further instructions on how to prevent future skin issues from occurring. Investigation determined AV did not receive showers on several occasions, and left in urine-soaked briefs on several occasions. AV sustained a skin rash to AV's thigh, buttocks and lower back and rash worsened over the course of exposure to urine. AV rash, skin injuries, and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
