Findings
The Alleged Perpetrator 2 failed to follow Witness 1's care plan to provide 1 to 1 supervision. The failure resulted in W1 being left unattended and engaged in a physical altercation with AV causing injury, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to ensure W1's care plan was followed which is a violation of Oregon Administrative Rules.
