Findings
The facility failed to follow Witness 2's (W2) care plan to ensure s/he is monitored at all times and not to be left unattended. The failure resulted in W2 wandering into the Alleged Victim's (AV) room and engaged in an unwitnessed physical altercation, causing unreasonable discomfort to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
