Findings
The Alleged Victim (AV) relies on the facility for his/her care. AV is on blood thinners and is known to bruise easily. According to an investigation, between January 10, 2024, and March 2, 2024, AV was found to have multiple skin tears and/or bruises of unknown origin, without adding additional interventions for skin integrity. The facility failed to implement interventions and/or properly care plan for AVs recurrent skin issues, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
