Findings
On or about February 29, 2024, Witness #1 (W1), Witness #2 (W2) and the Alleged Victim (AV) engaged in an altercation. All three residents are known to have altercations in the past. While the three residents were engaging, AV received a skin tear on his/her right hand. The facility failed to implement meaningful interventions to prevent altercations involving these three residents, placing residents at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
