- On or about April 1, 2024, the Alleged Victim (AV) was found to have skin breakdown on his/her sacrum. AV was admitted to the hospital on April 2, 2024, with moisture associated skin damage. AV was care planned for peri care after incontinence and was to have nystatin powder applied four times per day. None of this care was marked completed in AV's record. The facility's failure to mitigate AV’s skin damage caused unreasonable discomfort to AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
