Findings
Alleged Victim (AV) has a history of resident-to-resident altercations. On or about March 10, 2024, AV tried to grab another resident’s walker. This resulted in AV being hit on his/her arm. AV then hit Alleged Witness 1 (W1) in the back. The facility failed to provide a safe environment for AV, which put AV in risk of harm, which is violation of resident rights, is neglect of care and constitutes abuse.
