Findings
Witness 1 (W1) has a history of resident-to-resident altercations. On or about March 10, 2024, W1 tried to grab another resident’s walker. This resulted in W1 being hit on his/her arm. W1 then hit Alleged Victim (AV) in the back. The facility failed to provide a safe environment for AV, which put AV in risk of harm, which is violation of resident rights, is neglect of care and constitutes abuse.
