- ALleged Victim (AV) has a history of resident-to-resident altercation involvement due to approaching other residents in the dining room and attempting to to remove food or items from the table. AV's Service Plan does not list any behavior interventions for AV's know behavior of taking food from other residents in the dining room. Witness 1 (W1) has a history of agitation with other residents, including residents entering W1's space or touching W1's food; all of W1's behavior interventions require staff to observe the incident occurring, or beginning, and call for staff intervention to redirect or relocate W1. AV and W1 were involved in a resident-to-resident altercation on January 30, 2024 when AV approached W1's table and attempted to take cups off the table in front of W1. -W1 took the cups from AV's hands, resulting in AV hitting W1's hands, and W1 hitting AV's hands in return. AV did not have any immediate injury, but was found to have bruising on h/h left wrist on February 1, 2024; the bruising was attributed to the altercation on January 30, 2024. The facility failed to implement reasonable behavior interventions for AV, given both AV and W1's history, and to staff failed to follow the available interventions for W1, which resulted in a resident-to-resident altercation between AV and W1, with latent bruising to AV's wrist.
