Findings
Witness 1 (W1) has a known history of resident-to-resident altercations. On or about March 27, 2024, while in the living room, W1 hit Alleged Victim (AV) in the neck. AV then kicked W1 in the leg. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
