- The Alleged Victim relies on the facility for his/her care. According to an investigation, on or about March 31, 2024, Alleged Perpetrator 3 (AP3) did not follow the care plan, resulting in AV sitting in wet briefs for several hours, resulting in unreasonable discomfort. AP3's actions are considered neglect and constitutes abuse. The facility failure to ensure AV's care plan was followed, which is a violation of Oregon Administrative rules.
