Findings
The Alleged Victim (AV) relies on the facility for his/her care, is a fall risk, and is on blood thinning medication. According to an investigation, AV had multiple documented falls between approximately March 8, 2024, and April 17, 2024, some resulting in injury to include skin tears, abrasions, and a head injury. On at least one occasion AV was sent to the hospital for treatment. The facility failed to implement interventions to mitigate AV’s increasing fall risk, causing repeated unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
