Findings
The Alleged Victim (AV) had suffered a fall in March 2024, which resulted in a fractured hip. AV's care plan was updated on March 29, 2024, to check on AV frequently throughout the night. AV was to be checked multiple times per shift, but the care plan did not indicate at what time. On or about April 7, 2024, around 6:30 AM, AV was found face down on the floor, with soiled clothes and dried feces. It' is unknown how long AV was on the ground. AV sustained an abrasion to his/her knee. The facility's failure to ensure AV's care plan was updated so staff understood when AV should be checked on left AV at risk for serious harm. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
